[Solved] Help to fill form C4

Hello,

I have sent in court form C100 for the orders. I got a letter from court to fill and send form C4 as well.
I got confused when I was filling form C4. Can anybody help me please to let me know that what I need to write in these Numbers:

Form C4

3. The order your are seeking
State. A. the name of the person to be directed be the court to disclose relevant information as to the whereabouts of the child
B. Specific directions you would like the court to give as to when and how the information shall be disclosed to the court

4 The grounds of the application
State why you believe that
A. the court does not have adequate information as to where the child is
B. the person to whom the order is directed may have relevant information.

In section 3 they are asking you to give details of other people that will know where the mother/child is, for instance their family or friends, or their employer. Also public agencies such as the DVLAor the DWP if they are receiving benefits. Here's a link that might help you
