Anyone who has had to deal with the CSA has my deepest sympathy....
Last March my partner filled in a form for the CSA to appoint me his representative. Despite sending replies to my letters to him, for the last 6 months, they're now saying they've not got that form. if I phone up they won't discuss anything.
What I need is a blank copy of that form for the CSA. I cannot find it anywhere on the web anymore.
We've recently filled in one for the CMS, but the CMS does not have all the information - "All we have is an amount of arrears." They say talk to the CSA, the CSA say they can't discuss it with me.
Can anyone provide me with a link to that pdf or send me the pdf.
Again not the CMS form the CSA form.

Yes, you would think a formal letter would do the trick.
However, having made a subject access request in that format and had it.....er.....probably shredded shortly after they received it, I'm sticking with the forms.
The CMS form is similar or the same thing rebranded, so we've filled one of those in and, since they're in the habit of losing correspondence, sent multiple copies off....and another copy to the CMS just in case.
If you didn't already send it recorded, might be worth sending yet another one recorded, that way there is proof you sent it and they know that you are tracking it.
