[Solved] What should a CAO warning notice include?

Mine only states;

"WARNING: where a chilkd arrangerments order is in force:

if you do not comply with this contact order: -

a) you may be held in contempt of court
b) the court may make an order requiring you to undertake unpaid work."

My Exs solicitor wrote this and I didnt know and wasnt contacted just submitted to court, on the date submitted the day before the ex changed childs surname apparently everywhere.

Do I write to the court about this?

That's standard wording for the warning notice. It doesn't vary. Anything else such as name changes would be in the body of the order.
