Hi all,

I am looking to apply to the courts shortly but I would like to do everything in one swoop as it were, can you please tell me what I forms I need for a contact order, parental responsibility and to be on the birth certificate? Do I need to fill out more than the c100? Is there another form the caters for all of this? Will I have to pay a larger fee to do this? Not an issue just that I wish to get it right the first time round.

Hi TD

You'll need the C100 for the Child Arrangements Order for contact and the C1 form for the Parental Responsibility Order. Once you are granted the PR order you can then apply to have your name on the birth certificate.

If you submit both forms at the same time you will only be charged one fee of Β£215. If you are on benefits or a low income you can claim a part or full exemption from the court fee and you would need form EX160a to do this.
