Is there any chance such a thing exists?

My son has got to file and serve a statement in reply. All is going well, polishing it off - but don’t know how to format it.  Do you need to put ‘In the Family Court of’, respondent applicant etc?

Thanks Bill - I need a template for a respondent though.  Sorry I didn’t make myself clear.
