I just want to know about where or how I put 'ex-parte' on the c79 app tbh. And what about the respondent info/copies? Do I have to provide that if it's without notice?.
I'm planning to go for an enforcement tomorrow as the contact last week and weekend was breached. We are currently going through a section 47 child protection conference headed by the social worker who was coincidentally on annual leave last week and not back until Monday. I thought about going to court on Friday but advised myself the DJ would probably say wait until the SW is back.
So this is all probable atm but if I get the go-ahead from the SW in the morning I want to have all the paperwork prepped and ready to go - so I can have the midweek contact reinstated for Tuesday.
Also, when is good to ask for costs with enforcement or within-proceedings? This won't be the first time and I can prove she has cancelled contact without notice and later admitted to lying about the reasons why.

Here's a link to the explanatory leaflet CB5 which answers your questions.
You would need to tell the court you are requesting an urgent ex parte application.
As far as costs, there is a section on the application form that asks this so if you are out of pocket I don't see any reason why you shouldn't tick that box!
