Has anyone had success with submitting the C4 form or know of the costs involved?

Give the court a ring - they'll tell you the cost of appying.

I had to use this but submitted it with my C100 application for contact so there was no additional charge.
Assuming someone knows there wheareabouts of the child/mother it should come back.

My one did via CSA, The courts force them to disclose the address to the Courts, but the Courts wont tell you. You then just serve papers on the Court and they will send to the other person for you
