[Solved] Contempt of court application

Hi,

I got a Solicitor to do a contempt of court application and he completed a Contempt application form "rules 37.3 AND 37.4" is this the right form first of all?

 

He then said I need a notary to sign it? Is this correct...? which I have done....

He then said I need a "process server" which costs £100 - is that right that I need that as well....

The other thing is that the solicitor has kind of disappeared, so I wanted to ask him what I do when I get it from the process server? i.e. do I serve it only after then?
