Hello all!
This is my first day on my account so forgive me if i come across as a newbie.
I have a child who is 5 years old who I have unfortunately never met, I've gone through the mediation process and so has my child's mother, but it was deemed unsuitable (I'm not sure why though, the mediator didn't tell me, I can only assume it's because I've never met my child?).
I have been sent a certificate which states it's for page 9 of the C100 form. So, I am quite confused as to which form I need to apply to court with first, i was under the impression it would be C63, but it seems like it will need to be C100?
I was hoping someone would be be able to advise which form i will need to use, any advice will be much appreciated.

Hi, the mediator has given you permission to apply to court with that paperwork. Next step would be a c100 application, can do online:

Thanks for that! The bit which confuses me is the c100 form seems to imply I already have parental responsibility which I don't think I have as I'm not on the birth certificate
Will I not need to make an application with forms C1 - Application for an order & FM1 - Family Mediation Information and Assessment Meeting (MIAM)?
As in applying for parental responsibility first, and then if I do assume parental responsibility, I can then make a court application with form c100 regarding the child arragements?

hi, if ex refusing to put you on birth certificate, you can ask court to do it. thats form C63: declaration of parentage: https://www.gov.uk/government/publications/form-c63-application-for-declaration-of-parentage-under-section-55a-of-the-family-law-act-1986
you should be able to submit both c63 and c100 at same time. it's a long and slow process so better to apply together.

C63 costs £365
